Both sets of my grandparents lived in the city, but I had an aunt in Boston, and one who lived in Rockland County. That was just far enough away that those calls were charged at the in-state long distance rate, rather than being treated as either local (anything in the city proper) or long distance, which applied to calls to the rest of the United States.
